Payment before start
Payment is required before the assessment begins. Your partner does not pay separately — this single fee covers both questionnaires and the clinician-reviewed report.
Receipts & refunds
You'll receive a digital receipt on payment. Refunds are available before the questionnaire is started; once you begin, the fee is non-refundable.
Expedited scheduling
Need to be seen sooner? Submit a request via our Contact form. If granted, an additional $150 expedited-scheduling fee applies.
