This acknowledgment applies to the Educational Relationship Insight Report offered by Guiding Light Health Care Services, LLC. Please read it in full before purchase.

What this product is

• Structured questionnaires completed separately by each partner.
• A written summary of relationship history, communication, and conflict patterns.
• General educational information, discussion prompts, and exercises.
• Suggestions for obtaining professional assistance where you live.

What this product is not

• It is not therapy, counseling, or any form of behavioral-health treatment.
• It is not a diagnosis or a diagnostic service.
• It is not treatment planning or a clinical recommendation.
• It is not reviewed by a clinician licensed in your state.
• It does not create a clinician-client or provider-patient relationship.
• It is not a crisis or emergency service.

Separate acceptance by each partner

Each partner must create their own account and separately accept this acknowledgment, the Terms of Service, and the Privacy Policy before participating. One partner cannot accept on behalf of the other.

Privacy between partners

Individual questionnaire responses are private. The Educational Report presents themes and general education; it does not disclose one partner's individual answers to the other.

Payment and refunds

The Educational Report is $250 per couple, payable before the questionnaires begin. Refund and resolution requests are reviewed individually; see the Terms of Service.
